What Is Apolipoprotein B?

Apolipoprotein B, usually written ApoB, is the structural protein that forms each harmful, artery-clogging cholesterol particle in your blood — including LDL (the so-called “bad” cholesterol), VLDL, IDL and lipoprotein(a). Each of these particles contains exactly one ApoB molecule as its scaffold, so measuring ApoB effectively counts the total number of harmful particles circulating in your blood.

That matters because heart-disease risk is driven not only by how much cholesterol you carry, but by how many harmful particles there are. Two people can have the same LDL cholesterol yet very different particle numbers — and ApoB picks up that difference. That is why a high ApoB is important information even when a standard cholesterol panel looks reassuring.

ApoB measures the number of harmful particles, often a truer guide to risk than LDL cholesterol alone. Testing helps a cardiologist assess:

  • Your risk of coronary artery disease and heart attack from furring-up of the arteries
  • Your risk of stroke driven by atherosclerosis
  • A hidden risk that a standard cholesterol panel can underestimate — especially with raised triglycerides, diabetes or metabolic syndrome, where your LDL cholesterol can look at goal while your ApoB stays high

The test itself is quick and straightforward.

  • No fasting is needed — you can eat and drink normally and take your usual medicines before the test.
  • A member of our clinical team takes a single blood sample from a vein in your arm. It takes only a few minutes.
  • The sample is sent to the laboratory and your ApoB level is measured.
  • Dr Paul Lim reviews the result alongside your other cholesterol numbers, blood pressure, family history and lifestyle, and explains what it means for you.
  • If your ApoB is high, he will discuss how to lower your overall risk and, where you are already on treatment, whether it needs adjusting.

How is ApoB different from a standard cholesterol test?

A standard lipid panel measures the amount of cholesterol carried in your LDL, HDL and triglycerides. ApoB instead measures the number of harmful particles. With raised triglycerides, diabetes or metabolic syndrome, your LDL cholesterol can look at goal while your ApoB remains high — so the standard panel can underestimate risk and ApoB gives a more reliable picture. ApoB has to be requested as a separate test.

Do I need to fast, and how is the test done?

No fasting is needed for an ApoB test. It is a single blood sample taken from a vein in your arm by a member of our clinical team, and takes only a few minutes.

How often should ApoB be checked?

Unlike an inherited marker, ApoB can change with diet and treatment, so it can be repeated to track progress. If you are on cholesterol-lowering treatment, your cardiologist may re-check ApoB to see whether your particle number is coming down to target.

Can a high ApoB be lowered?

Yes. A high ApoB usually responds to the same measures that lower LDL cholesterol — improving diet, increasing physical activity, losing excess weight, not smoking, and, where appropriate, cholesterol-lowering medicines such as statins.
